• Parents must register child
• Bring child's birth certificate
• Correct name(s) of the Godparent(s)
• Stipend of $60, no refunds

• Can be two people, a male and female (friends or family), same sex is not permitted
• Can be one godparent
• If married, must be married through the Roman Catholic (cfr, Canon 842 s2) or be single.
• We require their marriage certificate by church or date of marriage, name of church, city and state
• Godparents who live in union or married civilly are not eligible to be godparents as they are not married by the Roman Catholic Church
• Must be 18 years of age
• Must have completed all the sacraments by a Roman Catholic Church. We require their First Communion and Confirmation certificates or date of sacraments, name of church, city and state.
• Should be practicing our faith

Must attend one pre-baptismal talk on Monday from 6:30-9:00PM prior to baptizing

• Upon registration you will be given your pass for the talk
• If one of you should not attend, the date of baptism is postponed until talk is taken
• If talk has been taken elsewhere, we will require proof with date of validation and parish seal
• If it happens that everyone already attended elsewhere, it is required for one of the parents to attend at St. Agnes of Bohemia
